National Telehealth Providers ("National Telehealth Providers," "NTP," "we," "us," or "our") respects your privacy and is committed to protecting the information entrusted to us.

This Privacy Policy ("Privacy Policy") describes how we collect, use, disclose, retain, and protect information when you:
Visit a National Telehealth Providers website or webpage that links to this Privacy Policy;
Enroll in or purchase a National Telehealth Providers membership, program, or service;
Communicate with National Telehealth Providers;
Use an NTP administrative, customer-support, billing, or enrollment service;
Interact with advertisements or communications relating to NTP;
Use services facilitated by NTP through participating healthcare, veterinary, pharmacy, laboratory, or technology partners; or
Otherwise provide information to us.

Collectively, we refer to these activities as the "Services."

This Privacy Policy should be read together with our Terms & Conditions and any other policies applicable to the Service you select.

1. IMPORTANT NOTICE ABOUT HEALTH INFORMATION
National Telehealth Providers facilitates access to healthcare and related services but does not itself practice medicine or make clinical treatment decisions.

Healthcare services made available through NTP are provided by independent healthcare organizations and licensed healthcare professionals.

Depending upon the Service selected, these organizations may include:
Licensed healthcare partners and their participating healthcare professionals or professional medical entities for certain Primary Care, Urgent Care, Behavioral Health, Dermatology, and related Care Services;
Licensed partners and their participating healthcare professionals or professional medical entities for certain men's health and Testosterone Replacement Therapy ("TRT") services;
Independent laboratories;
Independent pharmacies;
Other healthcare providers or healthcare organizations participating in NTP programs; and
Other healthcare technology or care-coordination organizations.

Veterinary Services may be facilitated through licensed veterinary partners and/or participating veterinary organizations or professionals.

These third parties may maintain their own privacy policies, Notices of Privacy Practices, patient consents, telehealth consents, and other privacy disclosures.

HIPAA and Protected Health Information
Certain information created, received, maintained, or transmitted in connection with healthcare services may constitute Protected Health Information ("PHI") under the Health Insurance Portability and Accountability Act of 1996 and its implementing regulations ("HIPAA").

Healthcare providers and professional medical organizations participating in NTP programs may be HIPAA-covered entities.

In certain circumstances, NTP may receive or maintain PHI on behalf of a healthcare organization as a business associate or in another capacity governed by applicable healthcare privacy law. Where HIPAA applies to information handled by NTP, we will handle such information in accordance with applicable law and any applicable Business Associate Agreement.

This Privacy Policy is not intended to replace a healthcare provider's Notice of Privacy Practices.

A provider's Notice of Privacy Practices describes how that provider may use and disclose PHI for treatment, payment, healthcare operations, and other purposes permitted by HIPAA.

Questions regarding a healthcare provider's privacy practices or requests involving medical records maintained by that provider should generally be directed to the applicable healthcare organization through its patient portal or designated privacy contact.

2. INFORMATION WE COLLECT
The information we collect depends upon how you interact with NTP and which Services you use.

We may collect information directly from you, automatically through our websites and technology, and from participating service providers and other third parties.

A. Information You Provide Directly
Information you provide may include:

Identification and Contact Information
Name;
Mailing address;
Billing address;
Shipping address;
Email address;
Telephone number;
Date of birth;
Age;
Gender or sex where relevant to a requested Service;
State of residence;
Physical location when healthcare services are requested; and

Account Information
If you create an account or use an online portal, we may collect:

Account identifiers;
Password or authentication information;
Account preferences;
Membership information; and
Account activity.

Passwords may be maintained by a third-party platform rather than directly by NTP.

Health and Healthcare-Related Information
Depending upon the Service, information provided to NTP or participating healthcare organizations may include:

Current or past health conditions;
Symptoms;
Medications;
Allergies;
Treatment history;
Prescription information;
Laboratory information;
Provider communications;
Health questionnaire responses;

Information relating to treatment eligibility;
Information relating to testosterone or hormone treatment;
Information necessary to coordinate prescription fulfillment; and
Other health-related information you choose to provide.

Clinical information may be collected directly by licensed partners, another healthcare organization, or a third-party healthcare platform rather than by NTP.

Veterinary Information
If you use Veterinary Services, information may include:
Your contact information;
Information about your pet or animal;
Animal species, breed, age, and sex;
Veterinary history;
Symptoms;
Medications;
Photos or other information relating to the animal;
Veterinary consultation information; and
Other information necessary to facilitate Veterinary Services.

Payment and Transaction Information
When you purchase Services, we may collect or receive information including:
Billing name;
Billing address;
Transaction amount;
Products or Services purchased;
Subscription information;
Payment status;
Transaction identifiers;
Refund or dispute information; and
Limited payment-method information.

Payment card information may be collected and processed directly by third-party payment processors, including Stripe or other payment processors used by NTP.

NTP may not directly receive or store complete payment card numbers, card security codes, or other information maintained exclusively by the payment processor.

Communications

NTP does not knowingly use PHI maintained in connection with clinical care for targeted advertising in a manner prohibited by HIPAA or other applicable healthcare privacy law.

We do not intentionally disclose the contents of medical questionnaires, laboratory results, prescriptions, diagnoses, provider communications, or other clinical records to advertising platforms for their own advertising purposes.

Public-facing website activity that does not constitute PHI may be subject to cookies and analytics technologies as described above, subject to applicable law.

Advertising and analytics practices involving consumer health data may also be subject to additional state privacy requirements described below.

6. INFORMATION WE RECEIVE FROM OTHER SOURCES
We may receive information from third parties involved in providing or supporting NTP Services.

Licensed healthcare partners;
Licensed veterinary partners;
Participating professional medical entities;
Healthcare providers;
Veterinarians;
Pharmacies;

Healthcare technology platforms;
Care-coordination organizations;
Payment processors;
Shipping providers;

Fraud-prevention providers;
Analytics providers;
Advertising platforms;

A patient is eligible or ineligible for a Service;

The exact information received depends upon the Service and our relationship with the applicable organization.

Coordinate access to veterinary providers;
Facilitate telehealth Services;

Process payments;
Manage subscriptions;
Process cancellations;
Process refunds;
Respond to billing questions;
Coordinate with participating third-party service providers; and
Perform other administrative functions associated with the Services.

Treatment, Payment, and Healthcare Operations
Where applicable and permitted by law, information may be used or disclosed to support healthcare treatment, payment, healthcare operations, care coordination, prescription fulfillment, laboratory services, and other healthcare activities.

Clinical treatment decisions are made by participating healthcare professionals and not by NTP.

Communications
We may use information to:
Respond to questions;
Send account notifications;
Send appointment or Service reminders;
Provide transaction information;
Provide billing notifications;
Communicate Service changes;
Provide customer support;
Provide important legal or privacy notices; and
Communicate on behalf of or in coordination with participating service providers where appropriate.

Verify payment information;

Investigate suspected misuse;

You may opt out of promotional email communications using the unsubscribe instructions included in the message.

You may opt out of promotional text messages using the instructions provided in the applicable message, such as replying STOP where applicable.

Opting out of marketing communications does not prevent us from sending necessary transactional, administrative, billing, security, or Service-related communications.

We do not sell or transfer text-message consent information to third parties for those third parties' independent marketing purposes.

9. HOW WE SHARE INFORMATION

A. Healthcare Providers and Healthcare Organizations
We may disclose information to:
Licensed healthcare partners;
Participating healthcare providers;
Professional medical entities;
Care-coordination organizations;
Laboratories;
Pharmacies; and
Other healthcare organizations involved in a patient's care.

Information may be shared as necessary to:
Coordinate healthcare Services;
Schedule or facilitate consultations;
Evaluate treatment eligibility;
Provide treatment;
Order laboratory testing;
Review laboratory results;
Issue prescriptions;
Coordinate prescription fulfillment;
Process payments;
Provide patient support; and
Conduct other treatment, payment, or healthcare operations permitted by law.

B. Pharmacies
When medication is prescribed, information may be provided to an appropriately licensed pharmacy as necessary to process, dispense, package, ship, or otherwise fulfill the prescription.

TRT prescriptions may be transmitted to different pharmacies depending upon the patient's location, pharmacy licensing, medication availability, provider requirements, and other factors.

Pharmacies maintain their own legal and privacy obligations regarding prescription information.

C. Laboratories
Information may be disclosed to independent laboratories or laboratory-service organizations when laboratory testing is ordered or coordinated.

D. Veterinary Service Providers
Information may be shared with licensed veterinary partners and/or participating veterinary professionals and veterinary organizations where necessary to facilitate Veterinary Services.

E. Payment Processors
We may provide information to payment processors, including Stripe or another participating payment provider, to:
Process payments;
Maintain recurring billing;
Issue refunds;
Prevent fraud;
Verify transactions; and
Respond to payment disputes.

Payment processors maintain their own privacy and security practices.

F. Service Providers
We may disclose information to vendors that provide services on our behalf, including:
Website hosting;
Cloud storage;
Customer relationship management;
Information technology;
Security;
Customer support;
Communications;
Email delivery;
SMS delivery;
Payment processing;
Fraud prevention;

We seek to limit information disclosed to service providers to information reasonably necessary for them to perform their services.

G. Legal Requirements and Protection of Rights

Where healthcare privacy laws impose additional restrictions, we will comply with those restrictions.

H. Business Transactions
If NTP is involved in a merger, acquisition, financing transaction, restructuring, bankruptcy, asset sale, or similar corporate transaction, personal information may be disclosed or transferred as part of that transaction, subject to applicable privacy and healthcare laws.

I. With Your Direction or Consent
We may disclose information when you direct us to do so or otherwise provide legally valid consent.

10. WHAT WE DO NOT DO WITH HEALTH INFORMATION
NTP does not sell PHI.

We do not authorize healthcare providers to disclose PHI to us for purposes unrelated to applicable treatment, payment, healthcare operations, administrative Services, or other legally permitted activities.

We do not knowingly disclose medical records, diagnoses, laboratory results, prescription information, or provider communications to data brokers for sale.

Where HIPAA or another healthcare privacy law requires a patient's authorization for a particular use or disclosure, the information will not be used or disclosed for that purpose without the required authorization.

11. THIRD-PARTY PATIENT PORTALS
NTP may direct users to patient portals operated or made available by third-party healthcare organizations.

For example:
Care Services may use a patient portal provided through licensed partners or their participating platforms;
TRT Services may use a patient portal or associated technology provided through licensed partners; and
Other healthcare or veterinary programs may utilize separate third-party portals.

These portals may collect information directly from you.

Information collected by a third-party patient portal may be governed by that organization's:
Privacy Policy;
Notice of Privacy Practices;
Terms of Service;
Telehealth Consent;
Patient Consent; and
Other applicable policies.

NTP encourages users to review those policies.

12. THIRD-PARTY WEBSITES AND SERVICES
Our Services may contain links to websites, portals, applications, or services operated by third parties.

This Privacy Policy does not govern the independent privacy practices of those third parties except to the extent NTP itself receives information from them and processes that information subject to this Privacy Policy.

We are not responsible for a third party's independent privacy or security practices.

You should review the privacy policy of any third-party service you use.

13. DATA SECURITY
NTP uses administrative, technical, and organizational safeguards designed to protect personal information against unauthorized access, acquisition, alteration, loss, misuse, or disclosure.

Depending upon the information and system involved, safeguards may include:
Access controls;
Authentication measures;
Encryption;
Secure transmission technologies;
Vendor security controls;
Employee or contractor access limitations;
System monitoring;
Security policies; and
Other reasonable safeguards.

Where NTP handles PHI subject to HIPAA, applicable HIPAA security requirements and contractual requirements also apply.

However, no method of electronic transmission, data storage, or information-security system can be guaranteed to be completely secure.

Users are responsible for maintaining the confidentiality of their account credentials and should promptly report suspected unauthorized access.

14. DATA RETENTION
We retain personal information for as long as reasonably necessary for the purposes for which it was collected, including to:
Provide Services;
Maintain accounts;
Process payments;
Meet contractual obligations;
Resolve disputes;
Prevent fraud;
Enforce agreements;
Maintain appropriate business records; and
Comply with legal, tax, accounting, healthcare, pharmacy, regulatory, and other requirements.

Different categories of information may be retained for different periods.

Medical records maintained by participating healthcare providers may be subject to separate medical-record retention requirements and are controlled by the applicable healthcare organization.

NTP may not have the legal authority or technical ability to delete records maintained independently by healthcare providers, pharmacies, laboratories, or other third parties.

15. DE-IDENTIFIED AND AGGREGATED INFORMATION
We may create aggregated or de-identified information that cannot reasonably be used to identify an individual.

We may use de-identified or aggregated information for purposes including:
Service improvement;
Business analysis;
Research;
Reporting;
Operational planning; and
Understanding trends.

Where information has been de-identified under an applicable legal standard, we will not attempt to re-identify the information except where permitted by law for purposes such as validating the de-identification process.

16. YOUR PRIVACY CHOICES
Depending upon your relationship with NTP and applicable law, you may have choices regarding your information.

These may include:
Updating account information;
Correcting inaccurate contact information;
Unsubscribing from marketing emails;
Opting out of promotional text messages;
Managing cookies through browser or website settings;
Requesting access to certain personal information;
Requesting correction of certain information;

Certain requests may be subject to exceptions.

Healthcare records maintained by a provider may need to be requested directly from that provider.

17. U.S. STATE PRIVACY RIGHTS
Residents of certain U.S. states may have additional rights under comprehensive state privacy laws.

Depending upon the law applicable to you and any relevant exemptions, these rights may include the right to:
Confirm whether we process your personal information;
Access personal information;
Obtain a portable copy of certain information;
Correct inaccurate information;
Delete certain information;
Opt out of certain sales of personal information;
Opt out of certain sharing or processing for targeted advertising;
Opt out of certain profiling;
Limit certain uses of sensitive personal information;
Appeal the denial of a privacy request; and
Not receive discriminatory treatment for exercising a privacy right.

Not every right applies in every state or to every category of information.

Information governed by HIPAA and information maintained by healthcare organizations may be exempt from certain state consumer privacy laws.

We may need to verify your identity before fulfilling a request.

An authorized agent may be permitted to submit certain requests on your behalf where provided by applicable law.

18. CALIFORNIA PRIVACY INFORMATION
If you are a California resident and the California Consumer Privacy Act, as amended by the California Privacy Rights Act ("CCPA/CPRA"), applies to your information, you may have additional rights.

We may make limited inferences from website activity or Service usage for purposes such as understanding user interests or improving our Services.

California Rights
Subject to applicable exceptions, California residents may have the right to:

Know the categories of sources from which information was collected;

Certain health information governed by HIPAA or other healthcare privacy laws may be exempt from some CCPA requirements.

Sale and Sharing
NTP does not sell PHI.

Depending upon the technologies used on our public-facing websites, certain online advertising activities may constitute "sharing" under California's broad definition of that term.

Where required by applicable law, NTP will provide mechanisms to exercise applicable opt-out rights.

19. CONSUMER HEALTH DATA PRIVACY
Certain state laws provide additional rights concerning "consumer health data" or similarly defined health-related information that may not otherwise be governed by HIPAA.

These laws may apply, depending upon the individual's state of residence and the nature of the information.

Consumer health data may include information that identifies or could reasonably be used to infer an individual's physical or mental health status.

Depending upon applicable law, NTP may collect consumer health data for purposes including:
Facilitating requested healthcare Services;
Determining which Service or provider is available;
Facilitating enrollment;
Coordinating provider access;
Facilitating laboratory Services;
Facilitating pharmacy fulfillment;
Providing customer support;
Processing payment;
Maintaining accounts; and
Complying with legal requirements.

We may disclose consumer health data to healthcare providers, healthcare organizations, pharmacies, laboratories, technology providers, and other service providers as reasonably necessary to provide Services requested by the individual.

NTP does not sell consumer health data without authorization where applicable law requires authorization or consent.

Where required by applicable consumer-health-data law, individuals may have rights to:
Confirm whether consumer health data is collected or shared;
Access consumer health data;
Obtain information concerning third parties with whom consumer health data has been shared;
Withdraw consent where processing is based upon consent;
Request deletion; and
Exercise other rights provided by applicable law.

Requests may be subject to identity verification and legal exceptions.

20. NEVADA RESIDENTS
Nevada residents may have rights regarding certain covered personal information under Nevada privacy law.

Where applicable, a Nevada consumer may submit a request to opt out of certain sales of covered information.

NTP does not sell PHI.

If Nevada law provides additional rights concerning consumer health data applicable to NTP's activities, Nevada residents may exercise those rights using the contact method identified below.

21. CHILDREN'S PRIVACY
NTP's general Services are not directed to children under 13.

We do not knowingly collect personal information directly from children under 13 through our general public-facing Services without appropriate authorization where required by law.

Certain healthcare Services may be available for minors when appropriately authorized by a parent or legal guardian and permitted by the participating healthcare provider and applicable law.

Information collected in connection with pediatric healthcare may be governed by additional healthcare privacy requirements and the applicable provider's Notice of Privacy Practices.

22. VETERINARY INFORMATION
Information about an animal generally is not treated as human health information under HIPAA.

However, information provided in connection with Veterinary Services may also identify the animal's owner and therefore constitute personal information.

We handle personal information associated with Veterinary Services in accordance with this Privacy Policy and applicable privacy law.

Veterinary service providers may maintain their own privacy policies regarding information they collect.

23. SECURITY INCIDENTS AND LEGALLY REQUIRED NOTICES
If NTP becomes aware of a security incident involving personal information, we will investigate the incident and take steps we determine appropriate under the circumstances.

Where applicable law requires notification to affected individuals, healthcare organizations, regulators, or other parties, we will provide notification in accordance with applicable legal requirements.

HIPAA breach-notification obligations will be followed where HIPAA applies.

24. BUSINESS-TO-BUSINESS AND PARTNER INFORMATION
NTP may also collect information relating to businesses, organizations, healthcare partners, vendors, contractors, and individuals acting in a professional capacity.

This information may include:
Business name;
Professional name;
Job title;
Business contact information;
Contract information;
Communications;
Payment information; and
Information relating to the business relationship.

We may use this information to manage partnerships, contracts, vendor relationships, business communications, billing, compliance, and other legitimate business activities.

Requests involving medical records maintained through the TRT program should generally be submitted through the applicable licensed partner or treating healthcare provider.

Veterinary Services:
Requests involving veterinary records should generally be submitted to the applicable veterinary provider or platform.

NTP will reasonably assist users in identifying the appropriate organization where possible.
