HEALTHCARE SERVICES
NTP facilitates access to healthcare and related services through independent third-party organizations.

Other healthcare or wellness services offered from time to time.

Care Services may currently be provided or coordinated through licensed partners and participating healthcare professionals.

TRT and certain men's health services may currently be provided or coordinated through licensed partners and participating healthcare professionals and professional medical entities.

All diagnoses, treatment recommendations, prescriptions, medication selections, dosage decisions, laboratory orders, and other clinical decisions are made solely by the licensed healthcare professional responsible for the patient's care.

Enrollment, payment, completion of an intake questionnaire, laboratory testing, or participation in a healthcare consultation does not guarantee that a particular treatment or prescription will be provided.

PRESCRIPTION MEDICATIONS
Prescription medications are available only when prescribed by an appropriately licensed healthcare professional who determines that treatment is clinically appropriate.

The availability of any particular prescription medication may vary based upon:

Clinical eligibility;
Laboratory results;
Provider judgment;
Patient location;
Applicable federal or state law;
Pharmacy licensing;

COMPOUNDED MEDICATIONS
Certain medications made available through programs facilitated by NTP may be compounded medications.

Compounded medications are not approved by the U.S. Food and Drug Administration ("FDA").

FDA does not review compounded medications before they are marketed to evaluate their safety, effectiveness, or quality in the same manner as FDA-approved medications.

Compounded medications may be prescribed when a licensed healthcare professional determines that a compounded preparation is appropriate for an individual patient and when compounding and dispensing are permitted under applicable law.

The decision to prescribe a compounded medication is made solely by the treating healthcare provider.

The dispensing pharmacy is independently responsible for preparation, compounding, quality controls, labeling, dispensing, and other pharmacy professional services applicable to the medication.

Patients should discuss the potential risks, benefits, and alternatives associated with any compounded medication with their treating healthcare professional.

FDA-APPROVED AND COMPOUNDED TESTOSTERONE
Testosterone is available in several FDA-approved drug products.

Depending upon the patient's prescription, treatment plan, pharmacy, availability, and other factors, a medication provided through an NTP-facilitated TRT program may be an FDA-approved commercially manufactured product or a compounded preparation.

The availability of a compounded option does not mean that the compounded medication itself is FDA-approved.

If a compounded testosterone preparation is prescribed, the FDA has not reviewed that compounded preparation for safety, effectiveness, or quality before it is dispensed.

Patients should contact their healthcare provider or dispensing pharmacist if they have questions regarding whether their prescribed medication is an FDA-approved commercial product or a compounded medication.

CONTROLLED-SUBSTANCE DISCLAIMER
Testosterone is classified as a controlled substance under federal law.

Prescribing testosterone requires an individualized clinical determination by an appropriately licensed healthcare professional and must comply with applicable federal and state controlled-substance laws and prescribing requirements.

NTP does not guarantee access to testosterone or continued eligibility for testosterone treatment.

Applicable legal and regulatory requirements may change and may affect the availability of telehealth-based TRT services.

PHARMACY FULFILLMENT
Prescription medications facilitated through certain NTP programs may be dispensed by one of multiple independent licensed pharmacies.

The pharmacy used for a particular prescription may vary based upon:
Patient delivery location;
Pharmacy licensing;
Medication availability;
Provider requirements;
Applicable law; and
Operational considerations.

Medication packaging, labeling, manufacturer, concentration, supplies, and appearance may differ depending upon the dispensing pharmacy and the medication prescribed.

OFF-LABEL PRESCRIBING
